COVID-19 Pandemic Pre-Requisites for Facility Access
- Take the IIPP/COVID-19 Return to Work Training. To access the training on the UC Learning Center you can use this link.
- Email a request to Courtnie Prather via cprather@ucsc.edu to coordinate a date and time at least 48 hours prior to anticipated visit.
- Complete and pass the COVID-19 Daily Symptom Check on the same day you intend on accessing the facilities.
- While on-site, follow the CDC COVID-19 safety protocol.
Facility Issue Report
- Is this issue an emergency after regular business hours? If so, please dial campus dispatch (831) 459-4861 to connect to police and fire departments.
- If you are reporting an issue that does not require emergency level attention, please continue submitting the report here.
Facility Access
Each staff member and volunteer will be granted varying levels of facility access according to their position and need for access to fulfill their position. Please confer with your direct supervisor about which facility areas you need access to in order to fulfill your duties as an Athletics and Recreation employee.
Facility Reservation Requests
Pease click here to submit a request to reserve facility space for your team/program. All facility reservation requests must be completed according to the following deadlines on a quarterly basis:

Step-by-Step Guide: Facility Reservation Requests

- Log in to campusrec.ucsc.edu using your CruzID@ucsc.edu email address.
- Select the hamburger menu at the top left of campusrec.ucsc.edu.
- Select "Facility Calendars" from the list of options.
- Select "Reserve a Facility" under the "Search Facilities" heading.
- Complete the requst form for the desired facility space.
- Pay special attention to the notes section of the form. This is where you may request recurring occurrences of the event, excpetions (may not want the reservation on a particular date) and when the recurring reservation should conclude.
Key Request Process
- Key requests are to be requested by program managers, to the Associate Director of Facilities.
- Individuals will be instructed to request keys through their Program Manager.
Method of Key Request Process
- Program Managers may requests keys via email, phone, or in person.
- Please include intended purpose and expected return date on request.
- Please submit requests at least 48 hours in advance of when keys are needed.
- Key requests may be submitted to the Facility Coordinator in the absence of the Associate Director of Facilities.
Key Distribution & Collection
- Program Managers will be responsible for distributing keys to the individuals under their area of supervision.
- All individuals will be required to sign off for their keys and will be held responsible for the key issued to them. The individual is required to return their keys to their Manager at the expiration period or will be held liable up to $50.00/replacement costs.
Permanent Program Issue
- Program managers may permanently check out keys under the program, and distribute and collect at their approval.
- Keys checked out to the program will be tracked in the key inventory database, under the Program Manager.
Key Responsibility
- Program managers are responsible to collect and return keys when employees and volunteers are no longer assisting their program.
- All keys must be returned at termination of employment.
Key Accountability and Audit
- The Associate Director of Facilities will perform an annual audit of all keys issued by program.
- Program managers will need to account for all keys issued under their program.
- Any keys not accounted for will be charged back to the responsible program FOAPAL.
