Injury Report

• This report should be utilized for any and all injuries and illness of staff, participants, patrons, visitors, etc. that participate in any of our programs, activities, or facilities.

Incident Report

•  This report should be utilized for any interpersonal issues including but not limited to theft, fighting, ejections, harassment, policy violation, etc.

Facility Issue Report

• Is this facility issue an emergency after regular business hours? If so, please dial campus dispatch (831) 459-4861 to connect to police and fire departments.
